    INSTRUCTIONS:

    If you have selected an Exam Preparation Boot Camp​ for Service Requested option (above), please proceed to answer questions 2a and 2b and skip questions 3 and 4 below.

    If you have selected a ​HS Math Tutoring for Service Requested option  (above), please proceed to answer question 3 and skip questions 2a, 2b, and 4 below. 

    If you have selected a Summer Robotics Camp (either full day or half day) for Service Requested option  (above), please proceed to answer question 3 and skip questions 2a, 2b, and 4 below. 

    Every applicable question must be filled out.
    Only answer questions 3a and 3b if you are selecting a subject tutoring option.
    Only answer questions 4 and 5 if you are selecting Summer Robo Camp or Rocket Science Camp.
